Poorly managed pain in the immediate postoperative period frequently extends the patient's length of stay in the post anesthesia care unit (PACU) and may lead to increased costs for an organization. Evidence-based clinical practice guidelines are emerging as one means to improve pain management practices. The American Society of PeriAnesthesia Nursing (ASPAN) Pain and Comfort Guidelines were developed to fill the gap for perioperative nursing pain management practices within the perioperative continuum (Krenzischek & Wilson, 2003). The overall objective of this project is to decrease PACU length of stay (LOS), related to pain, by improving the pain management performance of nurses in the PACU through the implementation of the ASPAN Pain and Comfort Guidelines. The specific aims of this project are to:Aim 1: Evaluate the impact of implementing the American Society of PeriAnesthesia Nursing (ASPAN) pain and comfort guidelines in a PACU Aim 2: Compare the differences in patient length LOS stay in the PACU pre and post implementation of the ASPAN pain and comfort guidelines. Aim 3: Determine the cost-effectiveness of implementing the ASPAN pain and comfort guidelines in the PACUThis pre and post implementation outcome project was conducted in a hospital that performs surgery on, approximately, 800 patients per month. Descriptive statistics was used to analyze the data obtained from this project. The findings show that the overall pain rating at time of discharge decreased post implementation; increase documentation of patient/family pain management education; and there was a decrease in PACU LOS from 2.95 hours pre-implementation to 1.70hrs post-implementation. Furthermore, expansion of this project may lead to a greater realization in cost savings related to a sustained decrease in the LOS.

The setting for this Clinical Nurse Leader (CNL) project was the Post Anesthesia Care Unit (PACU) at a level I trauma center in the Bay Area. The goal was to improve the discharge education performed by the PACU nurses to improve patient safety and decrease the chance of complications or readmissions to this hospital. With no clear instructions for how discharge teaching should be done, the nurses have many differing styles which leaves room for gaps in discharge planning. This influenced the implementation of the teach-back method during discharge planning in order to ensure proper education and increased patient understanding. A literature review revealed that the teach-back method has helped to decrease hospital readmissions and prevents complications patients may experience during home self-care. The Joint Commission, Agency for Healthcare Research and Quality, and the Institute for Healthcare Improvement all state that the teach-back method for discharge planning is best practice. Project data was collected through observation, microsystem assessments and through nurse and patient surveys. Interventions included nurse education at staff meetings, one on one in-service educations and teach-back handouts. The pre-intervention nurse survey (n=26) showed that 80% of the nurses were familiar with the teach-back method while only 36% of nurses used teach-back during discharge planning regularly. The patient survey (n=25) revealed that patients were able to retain 68% of the information presented during discharge education. One on one in-service education on the teach-back method is still being conducted at this time, with high nurse satisfaction. Follow-up surveys will commence when 100% of the staff have completed the in-service. The expected final outcome of this project is the implementation of the teach-back method during discharge planning by all nurses and increased patient understanding of discharge education.

In 2003 the American Society of PeriAnesthesia Nurses issued an evidence based practice recommendation in support of family visitation for all patients in the Post Anesthesia Care Unit. Family visitation in the PACU was recently introduced in a large hospital in the southeastern United States. The purpose of this study was to investigate the overall comfort levels of family members before and after a visit in the PACU. The hypothesis was that the overall comfort level of the family member would increase after a visit in the PACU. A quasi experimental pretest posttest design was utilized with a convenience sample of 50 family members recruited in the surgical waiting area. Data collection was accomplished with a shortened version of Kolcaba's General Comfort Questionnaire and a comfort rating on a 0-10 scale. The study findings indicated that there was no statistically significant difference in the overall comfort of family members after a visit in the PACU. The implications are that visitation should be continued with an increased emphasis on meeting the comfort needs of the waiting family member. Finding out what makes a family member comfortable and then providing that could possibly improve satisfaction and outcomes.

The use of capnography plays a significant role in patient safety while recovering from anesthesia and sedation.
